\Miami Surgery LLC's financial assistance program has a variety of ways to help reduce the patient’s financial responsibility for services rendered by Miami Surgery LLC. We
structure a balance offering the patient a reduced financial liability while still complying with required insurance obligations and Federal and state regulations.Contact our business office for additional information on our policies below. A few Surgeons, anesthesiologists, and other service providers (such as pathologists or laboratories) bill directly for their services separately from Miami Surgery LLC. and they may offer financial assistance program of their own. Contact them for further information.

Application Process
Our Staff will provide details for any of the elements within the financial assistance program if requested. The application process may require the patient to submit additional financial and household information to determine qualification. Application and Instructions under Forms Tab.

Payment Plans

We expect each patient to pay his/her estimated liability. In the event a patient is unable to pay in full, Miami Surgery LLC may offer a short term payment schedule after a down payment is made. For longer term financing schedule, a patient may need to secure their own financing from an outside source. Please consult with Miami Surgery LLC's business office for additional information and help.

Uninsured Discounts
Patients that are not eligible to receive these services paid for by their insurance or other party's payment sources may be able to receive an uninsured discount from our Miami Surgery LLC. Typically these discount do not apply for cosmetic surgery services. This discount would be a set percentage off the charges and is subject to change. When a patient’s services are found to be covered by insurance or other third party payment source, after the fact the uninsured discount may canceled.

Charity Care
Miami Surgery LLC has a charity discount policy that provides financial relief to patients who require medically necessary care and that do not qualify for state or Federal assistance. They must be unable to pay the remaining financial responsibility in part or in full to qualify. A patient must meet the companies policy's required minimum household income qualifications which are based on Current Federal Poverty Level Guidelines (which are revised annually). Supporting documentation must be submitted and is required to validate a patient’s status to qualify.
